After looking at the picture I think the shape there you are arguing about there might be his testicles as usually dragons have internal testicles and a pouch for the penis

plus if you look close there is this black magic goo stuff in his urethra up to his "balls" probably to prevent him from cumming in the bottom dragoness

Thanks I actually wanted to comment on this some time ago but I forgot it again ... well I did some roleplay with a dragon in the past they have a little unusual anatomy I would say
