Nah, urethral fucking is totally a thing, there's tons of videos of it out there, granted most of them are a lot smaller than a dick, but I've definitely seen some that are as big as the one in the pic.

Eh, depends on the size of the insertion. Anything bigger than an average sounding rod may cause permanent streatching, but anything smaller should be okay.
